A review spanning roughly 2,500 papers published between 2010 and 2025, randomised controlled trials and meta-analyses among them, reached a conclusion that is less exciting than most medical cannabis coverage and, we think, more useful: the trial evidence supports cannabinoids for a short list of specific conditions, and does not support cannabis or cannabinoids for most of what it is popularly promoted for.
Where the evidence is genuinely solid. Chemotherapy-induced nausea and vomiting is the clearest case, cannabinoids outperform some standard antiemetics here, and certain terpenes, limonene and beta-caryophyllene among them, appear to enhance the effect alongside THC. Multiple sclerosis spasticity has real trial support behind a standardised THC and CBD formulation. Chemotherapy-induced peripheral neuropathy is an active area of translational research into new cannabinoid formulations, promising but not yet settled. Pediatric irritability associated with Autism Spectrum Disorder is being studied specifically with CBDV, a minor cannabinoid distinct from the CBD sold in most retail products.
What that list does not include is the point. Anxiety, general chronic pain, sleep, mood, most of the conditions cannabis is marketed for in wellness contexts, sit outside this evidence base or rest on thin, conflicting data. That is consistent with what we lay out on our own Medical Use page: a short tier of licensed, trial-backed indications, a longer tier of reasonable-but-unproven evidence, and a very long tier of claims sold well ahead of any data.
Why we are covering a review instead of a single splashy trial. Individual studies get cited selectively by people arguing in both directions, reform advocates citing a positive result, prohibition advocates citing a negative one. A review that pools 2,500 papers and states plainly where the evidence is thin is a better basis for the honest-tiers approach this site tries to take than any single headline result. If your local news article claims cannabis treats something not on this list, this is the review to check it against.
